If you are a new patient, we encourage you to pre-register by printing our forms and completing them prior to your first appointment date. Click on the printable form(s) to the right. Completing your paperwork prior to arriving at our office will speed-up your registration process. You must bring with you two pieces of photo identification along with all insurance cards for processing. You will also be asked to provide us with a list of current medications, strength and dosage. Be sure to list ALL medications you are currently taking whether prescribed or over–the-counter. If you are an established patient but have had a change in your address, telephone number(s) or insurance plan, please print and complete the appropriate forms to bring with you to your next appointment. |
